1. The Environment (Protection) Act 1986 (29 of 1986) (EP act 1986) was passed by Parliament on 23rd May 1986 which came Into force from 19th November 1986 in whole of India.2. Notification under Section 6, 8 & 25 of EP Act 1986 was published in Gazette Vide S.O. 746 (E) dated 16th October 1997.3. Above notification invited objections from public with in 60 days from date of the publication of above said notification on the biomedical waste (Management & Handling) Rule 1998 (BWMH Rules 1998)

4. Objections received were duly considered.5. Central government notified above BWMH Rules on 20th July 1998.6. There are 13 section describing BWMH Rules 1998.7. There are 5 schedules appended to it under rule 5 & 6 of BWMH Rules 1998.8. There are three forms appended to it under Rule 8, 10 & 12 of BWMH Rules 1998.9. In first phase these rules are applicable to hospital/nursing homes in ‘1 Cities with population above 30 lacs. The deadline is 31st December 1999.10. The aim and object of rules is not to mix infected waste with uninfected waste and dispose off infected waste safely.

1. We are all aware that at present we are neither mixing infected waste with nor infectious waste.2. His mixed waste is going to municipal waste bin & finally to dumping ground of municipality.3. Infectious waste is generated in minute quantity. 4. Non infectious waste is huge in quantity.5. Mixing of two types of waste exposes everyone to the risks of contacting infections.6. Best common sense policy will be to segregate infectious waste from non infectious waste at the point of generation.7. Infections waste should be discarded in containers containing disinfectants. (1% sodium hypochlorite solution or Antisapril)8. Non infectious waste could be classified in to plastic waste eg. Wrappers, IV bottles, paper waste eg. Wrappers, pharma literature Glass waste eg. Injection bottles Metal waste eg. Caps of injection bottles. These can be given to Raddiwala for recycling.9. Infectious waste should not be given to raddiwala i.e. needles, syringes, catheters, gloves, IV sets, blood bags, etc. They also should not be mixed with general non infectious waste.10. Infectious waste should be packed in yellow or red plastic bag and sharp waste in white transparent jerry can.11. Bio-medical waste logo and label should be pasted on these containers.12. These containers should be weighed and given for final disposal to the yellow collection van with logo.13. This van should take these bags for final disposal to the disposal site for Incineration / Microwave / Autoclave.14. The Ash and other material should be put in municipal secured land fill.

lll BIO–MEDICAL WASTE MANAGEMENT IS COMMON SENSE

1. We are all aware that which is infected or potentially infections waste. ; .2. Do not mix with general waste.3. Disinfect at the point of generation.4. Collect in colour coded bag.5. Mark it with label & logo.6. Weigh & give it for disposal.7. Transport & disposal will be done by Excel.

IV. AUTHORIZATION

1. This first step towards biomedical waste disposal.2. You have to get a form number 1 on payment of As. 1001. from following address:

MAHARASHTRA POLLUTION CONTROL BOARD.Shri Chatrapati Maharaj Municipal Market,4th Floor, Mata Ramabai Road, Fort, Mumbai 400 001.

3. This form is to be filled in Quadruplicate (4 copies).4. Read Rule 8 of BWMH Rules 1998.5. The fees are prescribed on page no 2, which depends upon the capital cost of the institution. If the capital cost is less that Rs. 10 lacs then take out a draft of Rs. 2001- payable to ‘Maharashtra Pollution Control Board (MPCB)’.6. Generally hospitals & nursing homes need to seek authorization for generation & disposal.7. Other details are general in nature & there is no difficulty in filling in those details.8. Submit duly filled form with Rs. 200/- draft to (MPCB) office.9. Obtain receipts of form (Acknowledgement of Form No.1)10. Authorization will be granted in due course (generally 90 days).11. Last date of authorization is 31 st December 1999 in the cities with population over 30 lacs.12. Those who do not seek authorization & generate & dispose biomedical waste will be prosecuted under Environment Protection Act 1986 before Metropolitan Magistrate Courtsl Judicial Magistrate courts.

V. WHAT IS MEANT BY GENERATION OF BIO–MEDICAL WASTE

1. Anything which is not reutilizable or recyclable is waste.2. What comes in contact with body fruids, blood, sera, pus, stool, urine. etc. of patient and has potential to cause infection or spread infection is bio-medical waste.3. Live situations where bio-medical waste may be produced are:

a) Operation theatre/Minor 0. T.b) Labour roomc) Injection room/dressing roomd) Bed side administration of intravenous ids/blood.e) W.C.f) Kitchen Waste

4. What are reutilizable things:

a) 0. T. Instruments -Disinfect -Autoclaveb) Linen – Disinfect – Wash – Ironc) O.T. Linen – Autoclaved) Glass Syringes – Disinfect – Wash – Autoclave.

5. What are recyclable things -Raddiwala will take them:

a) Paper and plastic wrappers & bagsb) Paper & card boxesc) rv bottles plasticd) Injection grass bottlese) Injection metal caps.

These can be sold to raddiwala.

6. What is infected sharp waste:

a) Injection needleb) L.P. needlesc) Scalpvainsd) Scalpel blades.e) Ampoules

7. How to segregate infected waste in 0. T. and labour room:

a) Spread newspaper or plastic sheet on the floor of 0. T.b) Keep a bucket for placenta mops, guages, blood, IV sets, Allow blood bags, syringes. contactc) Filr 100 ml disinfectant in suction bottles. timed) Put linen in disinfectant after operation. ofe) Put instruments and gloves in disinfectant after operation 30f) Put newspaper or plastic sheet in disinfectant. Minutesg) Wash instruments & Autoclave –h) Wash linen & autoclavei) Cut or burn the needles.j) Put infected waste in yellow bags.k) Put sharps in Jerry can.

8. Similar technique to be applied in minor 0. T. & labour rooms.9. In injection & dressing room or bedside or consulting room.

a) Use needle cutters or burner & put in white jerry can.b) Scalp vain & IV set to be put in jerry can.c) Dressings & bandages & plaster cast if soiled put in yellow bags.d) Non infected material e.g. unsoiled dressing, plaster caste t wrappers, card boxes, etc. in black bag.

10. Use black bag for non infected waste.

a) Wrappersb) Papersc) Kitchen wasted) Coconut shellse) Fruits skins & peelings, etc.

VI HOW THEY SHOULD BE SEGREGATED

1. Put in yellow bags (infected) : Solid infected waste like, saline set, catheters, gloves, placenta, anatomical parts, cotton, gauze, bandages, soiled plastic casts, syringes, sani pads, diapers.2. Put in transparent jerry can (infected):

a) Needlesb) Cut needlesc) Scalpel bladesd) Saw blades.e) ampoules

3. Put in black bag (uninfected): Unsoiled bandages, plaster cast, kitchen waste. wrappers, paper boxes, plastic boxes, glass bottles, metal caps.4. Put in gunny bag & give it to raddiwala IV bottles, metal caps, glass bottles, paper boxes, pharma literature, paper waste, plastic & paper wrappers.5. Minimise infected waste by not allowing it to be mixed with non infected waste.6. Try and sell non infected waste to raddiwala except Syringes and l IV sets.

VII HOW TO PUT A BIO MEDICAL WASTE LOGO & STICKERS

1. Stick (non-washable) logo on each of Yellow bag Black bag White Jerry Can.

2. Cytotoxic dress logo (non washable).

VII HOW TO TRANSPORT

1. Close each of yellow. black & white Jerry can.2. Put a sticker & logo.3. Weigh each bag4. Enter in log book and enter type & category of waste date, weight. giver’s sign & receiver’s sign, date and time. .5. To be transported in special yellow biohazard van.6. Van should go to final disposal site in Incinerator/Microwave/Autoclave

IX FINAL DISPOSAL

1. This will done for us by Excel Industries2. They will disinfect the waste3. They will shred the plastic4. They will incinerate microwave or autoclave as per waste category.5. They will charge us Rs.75/- per kg. .6. Liquid waste will Rs. 100/- kg.7. They will review changes after 3 months and try to bring down cost.8. These changes include transportation costs
